Position Description :

The Advanced Practice Provider (APP) works as a member of a multidisciplinary health care team who will contribute to excellence in program administration, patient care, patient safety, research, quality improvement, and teaching.

The Department of Medicine - Division of Gastroenterology is seeking a Nurse Practitioner or Physician Assistant to support a multidisciplinary health care team, providing a broad range of diagnostic and therapeutic services for the patients of the General GI Outpatient services located in Menomonee Falls at Town Hall Health Center.

The APP will be expected to work in a shared practice model with one or more faculty members, as well as independently. The APP will be responsible for new and follow-up visits, in-basket management, TPN management and evaluating other acute patient issues.

Core business hours are Monday-Friday. May required evening or weekend hours in the future based on business needs. Start & end times vary based on business needs. Scheduling flexibility required to provide an appropriate level of patient care coverage.

Primary Function / Responsibility

• Collaborates with interdisciplinary health care teams in diagnosis and treatment of conditions requiring medical intervention in outpatient environment as a member of the gastroenterology team.
• Writes accurate and timely progress notes, procedure notes, and / or admission and transfer orders.
• Order, perform, and interpret screening and diagnostic procedures and tests, and appropriately communicates and documents information.
• Privileged to perform procedures beyond core as identified by the gastroenterology and hepatology services.
• Facilitates and communicates with patient, family and staff to promote continuity of care across the continuum of care.
• Attend required training and meets key regulatory requirements, including those of the privileging institution, CMS, and the Join Commission.
• Evaluates quality outcomes for the patients receiving care of this team through participation in the development of quality indicators and outcomes dashboard.
• Acquires information on the most current development and trends in the Physician Assistant field and / or Nurse Practitioner field, through educational programs, in-services, professional associates, and current literature

Position Requirements :

Minimum Education :

Advanced Practice Nurse : Master's Degree or higher in Nursing

Physician Assistant : Bachelor's Degree or higher in Physician Assistant Studies

Wisconsin RN License

Wisconsin Advanced Practice Nurse Prescriber License

Adult Care or Acute Care

Preferred Experience :

Experience with gastroenterology patients in an ambulatory setting
