Duties and Responsibilities:
Data Management :
- Manages and integrates the Department of Human Services database (Quickbase), the Homeless Management Information System (HMIS), eClinicalWorks (eCW), CRISP, and any other relevant databases, overseeing the collection, management, correlation, and evaluation of data.
- Oversees set up of HONEY’s data structure so that information can be easily and accurately gathered in support of the HONEY work plan and reporting requirements.
- Works with IT Specialist to ensure the EHR contains the necessary modifications to meet staff needs.
- Develops templates for the HONEY team in the EHR or other databases when needed.
- Completes weekly documentation review in EHR. Works with supervisors and staff to address data collection errors or delays in order to correct as soon as possible.
- Compiles required data in the EMR, Quickbase, HMIS, CRISP, and other databases to document client demographic data, as well as core program measures related to perinatal care, birth outcomes, housing, and program impact.
- Secures patient/client information and maintains patient confidence by completing and safeguarding medical records and keeping patient information confidential.
- Assists with data management to ensure compliance with grant deliverables.
Evaluation and Reporting :
- Assists in developing tools to monitor and assess programmatic successes related to referrals, trainings, and other functions.
- Prepares or assists with the preparation of a variety of regular and special reports to include the Health Resources and Services Administration (HRSA) Quality Improvement Fund-Maternal Health monthly report, Optum monthly reports, monthly data dashboards, etc.
- Creates tables, charts, and graphs, to display client data and grant deliverables.
- Prepares, organizes, analyzes and/or prepares materials in response to COH internal requests for program information and reports.
- Partners with HONEY team members to implement and monitor completion of a participant satisfaction survey, which will be conducted monthly.
- Attends monthly Community Advisory Council (CAC) meetings to collect quantitative and qualitative feedback from members.
- Meets regularly with Director of HONEY and team members to ensure completion of evaluation and reporting requirements.
- Contributes to the development of program policies, procedures, standards, and manuals in accordance with program goals, objectives and identified needs.
- Attends all appropriate departmental staff and clinical meetings, including Quality Management, Data Hub, DC Department of Human Services (DHS) team huddles, Health Services and Housing team meetings as needed.
- Attends external meetings and trainings as required by COH and funding agencies, including but not limited to the Department of Health, Department of Human Services, the Health Resources and Services Agency, and Optum.
- Performs other related duties as assigned.
Minimum Qualifications :
- Bachelor’s degree in social work, public health, public policy, or related field or equivalent experience required.
- If applicant has a BSW/MSW degree, LSWA/LGSW or appropriate social work license obtained within 90 days of hire.
- Familiarity and experience with quantitative and qualitative data collection and analysis methodologies required.
- Demonstrated cultural competence in communicating with underserved, diverse populations. Experience working with families preferred.
- Strong interpersonal, customer service, communication and organizational skills.
- Demonstrated capacity to input data accurately required. Experience working with EHRs or database system required; experience with eClinicalWorks preferred.
- Strong organizational skills required.
- Ability to work flexible hours, including weekends and evenings.
