[job_card.job_description]Full-time, Monday-Friday schedule Clinic hours : 7 : 30 AM - 5 : 00 PM (last patient scheduled at 4 : 15 PM) Outpatient primary care services - 100% outpatient Patient volume : 18 patients per day Small patient panel : Maximum of 450 patients Focus on senior patients with 3-5 chronic conditions Supported by a dedicated Medical Assistant and Scheduler Administrative time included in the schedule No nights, no weekends, no hospital call Access to in-house consulting specialists including cardiology, podiatry, acupuncture, tai chi, yoga, and nutrition services Door-to-door patient transportation available Board Certified Family Nurse Practitioner or Geriatric Nurse Practitioner Active and unrestricted Nurse Practitioner license in Ohio Minimum of 2 years of outpatient primary care experience Passion for value-based care and delivering high-quality outcomes for senior populations

Salary and Benefits
- Base salary : $140,000 annually
- Annual bonus potential : Up to $10,000 in the first year (increases with partnership opportunities)
- Paid time off (PTO)
