Conduct diagnostic evaluations to assess client needs and develop appropriate treatment plans. Oversee the implementation of home community care services, ensuring adherence to best practices and regulatory requirements. Ensure compliance with HIPAA regulations to protect client confidentiality and privacy. Collaborate with interdisciplinary teams to coordinate behavioral health services and support client progress. Monitor staff performance, providing feedback and conducting performance evaluations. Participate in ongoing professional development to stay current with industry trends and best practices Master's degree in Social Work (MSW) from an accredited institution. Proven experience in social work, mental health counseling, or related fields. Strong knowledge of patient care protocols, case management strategies, and diagnostic evaluation techniques. Excellent communication skills, both verbal and written, with the ability to engage effectively with clients and staff. Ability to maintain confidentiality and adhere to ethical standards in all aspects of work. Strong organizational skills with the ability to manage multiple priorities effectively. Possession of a Master's Degree in Social Work (MSW) from a school of social work fully accredited by the Council on Social Work Education (CSWE). Active Social worker license in California. Six (6) months (1000 hours) of social work experience in a health care setting. This experience may be gained from supervised field placement during the Master's in Social Work (MSW) program or from post-graduate social work experience.

- Medical Social Worker : 7am -5 : 30pm (4
- 10 hour shift), 40 hours / 4 days weekdays at Renal Center - Weekend coverage as per facility needs
- Medical Social Worker - Monday to Friday, 8 : 30am - 5pm (5
- 8 hour shift), FLOAT - Emergency Dept and Inpatient Med Surg Areas - Weekend coverage as per facility needs
- Medical Social Worker - Monday to Friday, 8 : 30am - 5pm (5
- 8 hour shift) - Out Patient Clinics - Weekend coverage as per facility needs.
