    • Successful Primary Care group hiring

    Location : LaPorte, Texas

    Outpatient Quality Care / Managed Care

    In Office -M-F : 8a-5p

    No weekends

    Low patient panel

    Great Work / Life balance

    Comfortable with use of Electronic Medical Records (EcW)

    Detect and manage chronic diseases

    Develop patient strategy to reduce hospitalization and ER visits

    Collaborate with Case Managers and Specialty Providers

    Educate patients on the value of compliance

    Compensation & Benefits :

    Competitive base salary of $230,000"$290,000 (depending on experience)
