Licensed Practical Nurse (LPN) at Long-Term Care Facility

Job Summary :

As a Licensed Practical Nurse (LPN) at [Facility Name], you will provide high-quality nursing care to residents in a long-term care setting. Your primary responsibility will involve assisting in the implementation of individualized care plans, monitoring patient health, and ensuring the overall comfort and well-being of our residents.

Key Responsibilities :

• Resident Care :
• Provide direct patient care, including administering medications and treatments as prescribed by physicians.
• Assist residents with daily living activities such as bathing, grooming, eating, and mobility.
• Monitor vital signs and report any changes in residents’ conditions to the nursing staff.
• Documentation :
• Maintain accurate and updated patient records, documenting care provided, observations, and any changes in resident conditions.
• Assist in the development and modification of individual care plans.
• Collaboration :
• Work closely with registered nurses, physicians, and other healthcare professionals to deliver comprehensive care.
• Communicate effectively with residents, families, and team members to ensure continuity of care and support.
• Health and Safety :
• Ensure a clean, safe, and comfortable environment for residents.
• Comply with all nursing standards, facility policies, and state regulations regarding patient care and safety.
• Education and Support :
• Provide education and support to residents and families regarding health management and wellness practices.
• Participate in care team meetings and training sessions as required.
• Emergency Response :
• Respond to medical emergencies and administer first aid or CPR as needed.
• Follow facility protocols for incident reporting and response.
Qualifications :
• Education :
• Completion of an accredited LPN program.
• Licensure :
• Current state licensure as a Licensed Practical Nurse.
• Experience :
• Experience in a long-term care setting is preferred but not required.
• Skills :
• Strong clinical assessment skills.
• Excellent communication and interpersonal skills.
• Compassionate, patient-centered approach to care.
