About The Position

Job Title : Home Health Physical Therapist Profession : Therapy / Rehabilitation Specialty : Physical Therapy Home Health Duration : 20 weeks Shift : Day Hours per Shift : 8 (08 : 00 - 17 : 00) Experience : Minimum of 1 year home health experience required; OASIS training preferred License : Valid state license required Certifications : BLS certification required Must-Have : Must have a valid driver's license and car for home visits. Must be COVID vaccinated or have an exemption form on file. Must have a flu shot or declination, with mandatory mask wearing while at work. Description : Client offers home health skilled visits. The position requires assessment and documentation skills. Knowledge of cardiopulmonary / circulatory systems is essential. Experience with various equipment and devices is necessary. Familiarity with orthopedics and neurology is required. Candidates must be able to create home exercise programs for patients. Education for patients and their families is a critical aspect of this role. Recognizing signs and symptoms of physical, sexual, or psychological abuse is necessary. Safety and injury prevention knowledge is essential. Experience with billing is required. The ideal candidate will have experience with patients in med / surg, cardiac, orthopedic, post-surgical, respiratory, and neurologic conditions. Home skilled visits range from 6-8 per day. The position allows for self-scheduling of visits and hours. Additional requirements include managing daily patient care effectively. Personal protective equipment, including masks, is required while working. Candidates must demonstrate proper time management skills. The role requires monitoring vital signs and performing wheelchair assessments. Individuals should be familiar with using adaptive devices and assistive equipment. Optimal candidates will have knowledge of specific therapy techniques, including myofascial release and functional capacity evaluation. This position involves working with adults and geriatric patients. Communication with interdisciplinary support staff is part of the role. The physical therapist must be capable of assessments and creating plans of care for patients. Skills in managing various physical conditions and injuries are required. The physical therapist will also need to conduct safety assessments in the home environment. Documentation and adherence to healthcare standards is essential for the role.
