Compensation $56,550.00 per year

Schedule : M-F 830a-430p

Medical or medical stipend. Dental, Vision, Benefit time, 401K

• Performs psychosocial assessments that are complete and timely : they identify major factors affecting care plan and coping and are completed within 5 days of admission.
• Develops care plans that are appropriate to meet the needs of individual patients and families and are carried out or revised appropriately.
• Solves routine care plan problems independently as appropriate.
• Advocates effectively for services from community organizations.
• Effectively elicits emotional expression in short-term counseling with patients and families.
• Meets established goals in a timely manner.
• Represents the organizations social work department in internal and external meetings, presenting accurate information effectively.
• Records planned and completed visits, phone calls and meetings in NDoc and other required media in a timely manner.
• Provides social work counseling to patient and family, with appropriate follow-up visit or phone call to ensure goals are met.
• Participates in interdisciplinary team meetings and / or individualized conferences.
• Adheres to employment policies as described in the employee handbook; maintains and respects confidentiality; adheres to all HIPAA regulations.
• Participates in all required in service education programs and meetings.
• Demonstrates compliance with the dress code policy by appearing well groomed, wearing ID badge while on duty and maintaining a professional appearance.
• Performs other duties as assigned.

Position Requirements

Required Skills / Abilities :

Education and Experience :

Required Education : Master's Degree in Social Work from a Graduate School of Social Work accredited by the Council on Social Work Education, Current N.J. License

Preferred Education :

Required Experience : One year of post-Masters Social Work or counseling experience.

Preferred Experience : Prior experience in a healthcare setting.
