Provides support around psychosocial stressors for patients and escalates crisis and high-risk calls as needed, to include utili Communicates to site mental health teams all calls received to ensure proper follow up planning as indicated. Identifies warning signs of increased risk and notifies appropriate parties with urgency needed to ensure safety of patient. Provides education to patients related to accessing resources, such as how to access mental health and medical services via sick slip an crisis protocols. Attends training and meetings as required. Documents details of each call, including the nature of the call, intervention provided, and disposition in the patient record. Demonstrates proficiency in all company software required for the position. Other duties as assigned. Master’s degree or higher in Social Work, Psychology, Sociology, Human Services or related field preferred.
