Co mmunity Healthcare of Texas is currently recruiting for an Field LVN.

POSITION SUMMARY

• Assist the RN Case Manager in the plan of care for the patient.
• Assists RN in case management of total plan of care of patient / support persons by coordinating with team members and communicating with IDG in accordance with organizational policies, procedures and regulations that define the scope of practice.
• Complete, maintain and submit accurate and relevant clinical notes regarding the patient's condition and care given. Record pain / symptom management changes / outcomes as appropriate.
• Administer medications and treatments as prescribed by the physician.
• Document nursing care and significant interactions with patients, support persons, and team members.
• Assess patient / family response to care and participate in follow-up with bereaved families with various types of contact.
• Manage assigned patients equally, regardless of severity, contagion, or complexity of diagnosis.
• Participate in on-call rotation
