Social Worker / Social Services Director – Skilled Nursing Facility

We’re looking for a caring and dedicated Social Worker to join our team as the Social Services Director. We’re a family-style, resident-centered nursing home where relationships matter just as much as clinical care. just looking for the right Director of Social Services to help direct and assist our residnets.

What you’ll do :

Welcome and support residents and families during admissions and throughout their stay.

Be a listening ear and advocate for residents’ social and emotional needs.

Help with care planning, counseling, and smooth discharges.

Work closely with our nursing and therapy teams to make sure every resident feels at home.

What we’re looking for :

Bachelor’s degree in Social Work (BSW); MSW a plus.

License / certification as required by the state.

Someone compassionate, approachable, and a real team player.

Experience in a nursing home or healthcare setting is helpful, but we’re open to the right fit.
