Minimum year experience required, FIRST TIME TRAVELERS - OK!

Previous experience as an LCSW in a psychiatric or behavioral health setting, strongly preferred.

BLS required

NC licenses required

EPIC

Transition of Care Department Clinical Social Workers handle disposition planning, Social Determinants of Health (SDOH), domestic violence, substance abuse and homelessness. This position specifically will specialize in SNF placement and home health / DME arrangement for adult Neurology (primarily Stroke and Seizure) patients. There are opportunities to assist with other patient populations, however the job remains the same.
