A unique opportunity for a mission-driven physician leader to shape the future of value-based senior care. This role combines executive-level leadership with a limited clinical practice, ensuring alignment between strategy and patient care delivery.
Position Highlights :
- Split role : 80-90% leadership | 10-20% clinical practice
- Lead multiple primary care centers supported by comprehensive, multidisciplinary teams
- Oversee programs in chronic disease management, prevention, care transitions, and disparities reduction
- Mentor and coach medical directors and local providers across the market
- Collaborate with operations, analytics, and technology teams to advance population health strategies
- Deploy care pathways and risk stratification tools tailored to a fully capitated, value-based model
- Ensure clinical quality, patient experience, and cost optimization
Compensation & Benefits :
