Evening Social Worker

Part-time, Non-Exempt position

Shift : 5 PM - 9PM, Monday – Friday

Location – Syracuse, NY

Van Duyn is seeking a dedicated Part-Time Social Worker to join our dynamic team! This role is primarily responsible for completing psychosocial assessments and care planning for new admissions and supporting residents and families by addressing concerns during the evening shifts. This position is supervised by the Director of Social Services and works in tandem with the remainder of the Social Services Department.

Education & Experience :

Masters of Social Work (required)

Experience in long-term care, rehabilitation, or skilled nursing or related setting (preferred)

Experience in admissions intake / discharge planning (preferred)

Knowledge of DOH and CMS guidelines (preferred)
