Responsibilities
As a Nurse Practitioner, you will conduct annual risk assessments in residential settings or via telehealth, within the designated scope of practice as guided by the Collaborative Physician.
Key Duties Include :
Conduct comprehensive in-home health risk assessments to identify acute and chronic health conditions and evaluate physical, mental, and social needs during each visit.Gather patient history, conduct examinations, and provide diagnoses.Document patient visits in accordance with NCQA standards.Measure patient vital signs when necessary.Provide case management referrals and coordinate communication with Primary Care Providers (PCPs).Engage with patients, caregivers, agency nurses, and other providers for accurate diagnosis and care.Perform clinical duties while adhering to OSHA Universal Precautions.Maintain strict patient confidentiality.Participate in required meetings and training sessions and contribute to committees as requested.Engage in professional development activities and uphold necessary professional licenses and affiliations.You Will Collaborate With :

Required Knowledge, Skills, and Experience :
Active / unrestricted Nurse Practitioner license in relevant statesBoard certification from ANCC, AANP, or NCCPACurrent CPR CertificationEnrollment in Medicare / Medicaid programsValid driver's license and excellent driving recordStrong EHR skillsPreferred Knowledge, Skills, and Experience :
Experience or training in geriatricsExcellent teamwork and relationship management skills with patients, medical staff, and the publicConditions to Consider :
