As a Social Worker, you will provide essential social work services to patients and their families, serving as a key member of a compassionate healthcare team. Your role will involve educating patients, families, and healthcare staff on benefits, available community resources, and counseling referrals. You will also assess referrals and collaborate with other healthcare professionals to ensure comprehensive care. Your responsibility includes advocating for patients in a supportive and sensitive manner.

Primary Responsibilities :

• Conduct assessments of patient and family psychosocial needs, developing tailored care plans in collaboration with the healthcare team.
• Connect patients and families to community resources such as financial assistance, transportation, and support groups.
• Thoroughly document patient / family status, treatment plans, goals, interventions, and evaluation outcomes in medical records.
• Act as a patient advocate and a liaison with healthcare providers, families, and insurance companies to ensure seamless care continuity.
• Facilitate discharge planning that meets the established goals of the treatment plan.

Your compensation will reflect various factors, including local labor markets, experience, education, and certifications. Along with your salary, you'll benefit from a comprehensive benefits package, incentive and recognition programs, equity stock purchase options, and 401k contributions. All benefits are subject to eligibility requirements. The salary for this role ranges from $58,800 to $105,000 annually based on full-time employment. We adhere to all minimum wage laws as applicable.
