Bilingual RN Case Manager

A company is looking for a Bilingual Clinical Guide : Case Manager (Gold Care).

Key Responsibilities

Develop long-term patient relationships and conduct holistic assessments to identify patient needs

Create and evaluate care plans in collaboration with interdisciplinary teams and caregivers

Support chronic disease management and ensure smooth transitions of care for patients

Required Qualifications

An unrestricted compact RN license

RN Complex Case Management experience

A minimum of 5 years of direct clinical experience in acute care

Strong knowledge of interdisciplinary teamwork

Comfort with participating in interdisciplinary discussions
