Home Health Physical Therapist

10-15 visits / week

What Youll Do

Deliver direct patient care and evaluate functional needs to design effective treatment plans.

Develop and implement personalized care plans based on thorough assessments and patient goals.

Collaborate with physicians nurses and other clinicians to ensure optimal outcomes.

Guide and direct support staff to help patients meet their functional goals.

Maintain accurate timely documentation of evaluations goals and progress notes.

Promote Elara Carings mission and represent physical therapy services within the community.

What Youll Bring

Graduate of an APTA-accredited Physical Therapy program

Current unrestricted PT license in the state of Michigan

At least 1 year of experience in a healthcare setting

Reliable transportation valid drivers license and current auto insurance

Ability to travel up to 50% within your coverage area

Physical ability to lift transfer and assist patients (50100 lbs)
