What You’ll Do
Manage a caseload of patients with individualized care plans
Perform comprehensive assessments and develop care plans
Deliver skilled nursing care, treatments, and medication administration
Educate patients and families on managing conditions and improving quality of life
Collaborate with physicians, therapists, and the care team to ensure safe, coordinated care
Accurately document visits to maintain compliance and care continuity
What We’re Looking For
Current RN license in Oklahoma
Minimum of one year of nursing experience (home health preferred, but not required)
Strong clinical judgment, organization, and communication skills
Compassionate, dependable, and patient-focused
Valid driver’s license, reliable transportation, and auto insurance
Pay & Benefits
Full-Time : $40 / visit
SOC (Start of Care) : $100
ROC (Resumption of Care) : $75
Recertification : $60
