Key Responsibilities

• Conduct thorough evaluations of clients in their home environments and develop individualized therapy programs with measurable short‐ and long‐term goals.
• Deliver treatments aligned with the physician's plan of care, documenting progress, reassessments, and discharge summaries in a timely and accurate manner.
• Act as a liaison with physicians, managed care providers, aides, and other members of the interdisciplinary team to ensure efficient coordination of care.
• Supervise a Physical Therapy Assistant (PTA) : assess their ability to perform interventions, ensure required co‐signatures, and monitor ongoing performance.
• Provide written home‐exercise or care programs to patients / caregivers, give education based on their needs, and foster a respectful therapeutic relationship to promote goal achievement.
• Participate in discharge planning, respond promptly to voicemails / messages, update schedules in the system, and document communications across the care team.
• Meet productivity standards consistently, adhere to organizational competencies and standards of behavior, and carry out additional duties or projects as assigned.
• Physical demands : lift / push / pull at least 50 lbs and stand for a minimum of 8 hours per day.
