Washington LPN or RN Nurse

A company is looking for a Primary Care Nurse (RN or LPN).

Key Responsibilities

Provide safe, timely, and patient-centered care in collaboration with primary care providers

Manage triage calls and messages, supporting patient care coordination

Drive quality outcomes for an attributed panel of patients

Required Qualifications, Training, and Education

For LPN : Diploma in Practical Nursing and active LPN license

For RN : Associate of Applied Science in Nursing or BSN, or 4 years of equivalent experience, and active RN license

2-3 years of relevant experience preferred for both RN and LPN
