Sparx Support Services is a fast-growing Care Management Agency (CMA) providing Health Home Serving Children services across New York State. Our mission is to support children and families through coordinated care, advocacy, and access to critical resources. We partner with providers, schools, therapists, and community organizations to ensure every child receives comprehensive support.
Position Summary
We are seeking compassionate, detail-oriented Care Managers to join our expanding CMA program. The Care Manager will work directly with children (021 years old) and families who qualify for Care management services, complete assessments, develop Plans of Care, coordinate providers, and ensure compliance with NYS DOH Health Home Serving Children (HHSC) standards.
Responsibilities
- Complete intake assessments, CANS-NY evaluations (upon training), and Plans of Care
- Provide ongoing care coordination for children with medical, behavioral, or social needs
- Communicate regularly with parents, schools, providers, therapists, DOH, and Health Home partners
- Maintain accurate, timely documentation in FCM, MAPP, and internal systems
- Track monthly contacts, outreach attempts, and compliance requirements
- Help families access Medicaid services, therapy, community programs, and supports
- Attend required meetings, trainings, and supervision
- Manage a caseload of 2040 children (based on full-time/part-time status)
- Uphold all DOH guidelines and HIPAA standards
Qualifications
- Bachelors or Masters degree
- Experience working with children and families in community or school settings
- Strong communication, documentation, and organizational skills
- Ability to work independently and meet deadlines
- Comfortable traveling to homes, schools, and community settings as needed
- Bilingual preferred but not required
