Responsibilities

• Serve as primary nurse leading the collaboration of all disciplines for well?coordinated patient care prior to admission and procedure / surgery.
• Obtain patient history and current medications, ensuring nursing assessment is completed and standard of care initiated.
• Obtain and / or coordinate pre?admission diagnostic testing (EKGs, labs, CXR, etc.), analyze pertinent data, communicate findings, and document information accurately.
• Initiate and document pre?procedure / operative instruction and discharge teaching, involving patient and family / significant other.
• Work telephonically or in?person with the patient.
