Position Summary
The Community Liaison serves as the primary representative of our agency within the community, regularly visiting physician offices, hospitals, skilled nursing facilities, and other referral sources. This is a field-based position that requires frequent driving to meet with partners, build relationships, and support timely patient referrals.
The ideal candidate is personable, proactive, and committed to ensuring that patients and referral sources receive outstanding support throughout the care transition process.
Key Responsibilities
Drive to and meet with referral sources across our service areas
Build and maintain strong relationships to generate and sustain patient referrals.
Educate healthcare professionals about our home health services, eligibility criteria, and the benefits of skilled home care.
Work with the intake / case management team to ensure smooth transitions from hospital or facility to home.
Follow up with referral sources to ensure satisfaction and resolve concerns promptly.
Track referral activity and prepare regular reports for management.
Qualifications
Prior marketing experience is a plus.
Excellent interpersonal, presentation, and communication skills.
Must have a valid driver’s license, clean driving record, and reliable personal vehicle.
Organized, self-motivated, and able to meet deadlines and referral goals.
