Facility: PACE Program. Each of the facilities is easily accessible, offers gardens, soft lighting, colorful artwork, and winding hallways to create a warm, welcoming experience. There is a strong sense of community among participants and employees.

Opportunity: PACE Program Nurse Practitioner

*Full Time Permanent * Schedule: Monday - Friday
* Responsible for the medical care of program's enrolled geriatric participants across a variety of settings, including but not limited to the Day Center clinic, participants’ homes, the nursing home, assisted living facilities and personal care boarding homes. Most participant visits occur at the clinic, though some physicians may specialize in nursing home care. Close coordination of care with hospital-based providers is essential. Physical attendance at facility is essential to performing the job duties.
* Bilingual Spanish a plus (NOT required)

*Prior experience with geriatric patients

*Active CA License of Willing to Obtain
