RN Telehealth Triage

A company is looking for a Telehealth Triage RN for the 2nd Shift.

Key Responsibilities

Perform clinical triage to assess member health concerns and direct care appropriately

Engage members in clinical discussions to understand their needs and guide them in care decisions

Coordinate care, including scheduling appointments and follow-up for preventative and chronic care

Required Qualifications

RN, ADN, or BSN degree

Active RN license in a compact state

5+ years of direct patient care experience, preferably in family medicine

Triage nursing experience desired

Experience in population health or disease management required
