Vivo HealthtStaff is seeking an experienced Dietitian to work hyrid for a non-profit organization for the elderly in San Francisco, CA.

Responsibilities :

• Visits and observes participants in order to evaluate their nutritional needs. Reviews diet orders in charts and confer with physicians, nurses, and participants' family, when necessary. Completes dietary assessment and quarterly progress note for participants as required.
• Works with and participates actively in the interdisciplinary team regarding diet and nutritional problems and management, including participant care plans, problem-solving and case review. In collaboration with the Center / Program manager and other specialists promotes staff development in the area of nutrition.
• Provides nutritional education to participants, family, and staff as needed; coordinates use of community resources. Provides information to the Nutrition Services Manager concerning the preparation of meals for participants including menu planning, diet textures, and nutritional planning.
• Fulfills all requirements, such as reporting, keeping statistical records, making entries into charts and attending meetings as required.
