Benefits offered to Per Diem / Part Time Team Members :

401(K)

Pay range (depending on experience) :

SOC / ROC : $125

Revisits : $60

Recerts : $80

Schedule / Shift : Part Time, Saturdays and Sundays, 8AM-5PM

Territory / Location : Phoenix and surrounding areas

Responsibilities :

• Conduct thorough assessments of patients and their families to determine home care needs.
• Provide comprehensive physical assessments and document the history of illnesses.
• Develop and update individualized care plans with active participation from patients and their families.
• Identify discharge planning needs and ensure they are addressed before patient discharge.
• Offer counseling to patients and families to address nursing and related health needs.
