Washington Licensed NICU RN Case Manager

A company is looking for a NICU Case Manager, RN - Remote in WA.

Key Responsibilities

Engage members to complete comprehensive needs assessments covering medical, behavioral, and socioeconomic needs

Develop and implement person-centered care plans to manage chronic health conditions and promote wellness

Collaborate with internal care teams and community resources to implement care plans and support member self-management

Required Qualifications

Current, unrestricted RN licensure in the state of Washington

2+ years of NICU clinical experience

1+ year of experience with MS Office applications

Reside in the state of Washington

Designated workspace and secure high-speed internet access
