JOB SUMMARY / OBJECTIVE :
The Licensed Practical Nurse (LPN) provides comprehensive care to all assigned patients under the supervision of the Nurse Manager, in accordance with the Maryland State Nurse Practice Act and agency policies and procedures. The LPN is responsible for delivering quality care, making clinical decisions, and adhering to regulations set forth by the Maryland Department of Health and Mental Hygiene.
ESSENTIAL DUTIES AND RESPONSIBILITIES :
- Patient Follow-Up : Conduct follow-ups for patients requiring medical attention based on reviews of medical files and documentation, ensuringtimelycare.
- Medication Administration : Administer medications to clients as prescribed, ensuring safe and secure storage of all medications per state and federal guidelines.
- Regulatory Compliance : Maintain current knowledge of state standards for nursing practice in an intermediate care facility andensure compliance at all times.
- Nursing Assessment : Perform comprehensive nursing assessments for each client upon admission and develop discharge care plans for ongoing medical conditions.
- Staff Orientation : Assistin the orientation of nursing staff as directed by the Nurse Manager, fostering a culture of compliance and patient safety.
- Record Maintenance : Ensure all medical forms and clinical records are completed accurately andin a timely manner,in accordance withHIPAA regulations.
- TB Testing Records : Maintainaccuraterecords of tuberculosis testing for all employees, following state health regulations.
- Patient Documentation : Review and complete necessary patient paperwork asrequiredtomaintaincompliance with healthcare standards.
- Incident Monitoring : Monitorand address client complaints and incidents, following agency and regulatory guidelines for complaint tracking.
- Infection Control : Oversee and report on Infection Control / Surveillance activities to ensure a safe environment for clients and staff.
- Client Care Records : Maintaina comprehensive system of client care records, reporting forms, and service datain accordance withstate and federal requirements.
- Service Coordination : Ensure all client care services are coordinated and integrated appropriately among multidisciplinary teams.
- Clinical Documentation : Keep necessary clinical documentation from physicians, nurses, and other healthcare providers current and accessible.
- Collaboration : Work collaboratively with other healthcare professionals to provide holistic care.
- Additional Duties : Accept and perform tasks assigned by the Medical Director, Executive Director, or theirdesigneethat are appropriate to this role.
- Meetings : Attend required meetings andtrainingstoremaininformed and compliant with best practices.
- Other Duties : Perform all other duties as assigned.
REQUIRED EDUCATION, CERTIFICATIONS AND / OR EXPERIENCE :
High school diploma or GEDrequiredValid Maryland LPN licenserequiredBLScertificationisrequired.REQUIRED KNOWLEDGE, SKILLS AND ABILITIES :
Abilityto conduct ongoing chart reviews toprepareaudits and ensure compliance with state and federal regulations.Demonstrated commitment tomaintainingclient confidentiality and respecting clients' rights.Strong adherence to professional standards of practice, including punctuality, reporting protocols, and professional conduct.Consistent execution of monitoring tasks as outlined in Infection Control Policies.Ability to train and mentor staff in medication administration and clinical procedures.Skilled in coordinating client medical referrals and orienting clients to appointment procedures.Proficiencyin learning andutilizingin-house software for documentation and reporting.Flexibility to work evenings and weekends as needed.Commitment to perform all other duties as assigned.Excellent patient care skillsPersonal belief in recovery and the recovery process.Excellent written and verbalcommunication skills, with the ability to interact effectively with staff, clients, and the public.High energy, flexibility, and strong interpersonal skills to work effectively within a team.Ability to engage clients flexibly based on their individual needs.Respect for and maintenance of professional boundaries with all individuals.Ability to follow written and verbal instructions accurately.Proficiencyin reading, understanding, and speaking English.Background : This position requires successful background and drug test completion
Physical Requirements :
Prolonged periods of sitting at a desk and working on a computer.May require prolonged periods of standing,moving,and bending.Must be able tolift upto 75 pounds at times.SALARY AND BENEFITS :
The compensation for this position will vary depending on factors such as your location, skills, job-related knowledge, and experience. The compensation package may also include incentive compensation opportunities in the form of a discretionary annual bonus.
