The role

As a Nurse Navigator, you'll serve as a key clinical touchpoint for our clients-handling both subacute and acute needs-through a mix of telehealth, in-person visits, and care coordination. You'll work across care segments, stepping in as a steady, skilled guide for clients and a clinical resource for our nurse contractors.

This hybrid position is intentionally flexible : you'll rotate between remote work, in-person client visits, and on-site collaboration at our Phoenix headquarters. It's ideal for someone who values autonomy and variety while delivering high-quality, high-touch nursing care.

What you'll do

• Perform in-person visits for acute care assessment and planning.
• Manage complex care needs and maintain high levels of service quality.
• Support nurse contractors with clinical concerns; serve as a clinical resource.
• Lead quality assurance through chart audits and continuous feedback loops.
• Adapt to changing healthcare environments and client needs.
• Manage administrative tasks related to documentation, sending docusign contracts and billing.
