What You’ll Do

Respond quickly to new inquiries and referral leads

Build and nurture referral relationships across North Denver

Promote and support Sensi AI, our virtual care assistant

Log and track activity in HubSpot

Complete assessments, intakes, and care plans in WellSky

Support families through consistent communication

Facilitate caregiver–client introductions

Partner closely with the Executive Director on account growth

Deliver last-resort patient care when needed to prevent service gaps

Represent Home Matters with heart, professionalism, and integrity

Schedule & Standards

Monday–Friday, starting at 8 AM (7 : 30 AM daily huddle)

Accurate, complete, and timely documentation

Return client paperwork within 1 business day

Uphold our Purpose Driven & Nurse Guided Care philosophy

Participate in monthly on-call rotation

Performance Expectations

• Daily : urgent updates, follow-ups, APFM leads, HubSpot activity, daily summary
• Weekly : 17 referral visits, 1 in-service, 1 workshop, 1 industry event, 1 networking event, 3 assessments, 2 new sign-ups
• Monthly : MBR meeting, KPI reporting, Care Academy modules

Compensation : $75,000 base + commission
