Physical Therapist Assistant, PTA - Per Diem (Home Health)

We are seeking a passionate and skilled Physical Therapist Assistant to join our team on a per diem basis, providing exceptional care directly in patients' homes in Peoria, Arizona. This role requires a dedicated professional who is committed to improving patients' quality of life through personalized physical therapy services.

Key Responsibilities

• Delivering skilled physical therapy interventions, including exercises and stretches, aimed at enhancing patients' mobility and function.
• Creating and executing individualized treatment plans tailored to each patient's specific needs and goals.
• Collaborating with a multidisciplinary team of healthcare professionals to ensure holistic and coordinated patient care.

Required Qualifications

• Current Physical Therapist Assistant license in the State of Arizona.
• Valid AZ Fingerprint Clearance Card.
• Up-to-date BLS (CPR) / ACLS certification.
• Recent Negative TB test result and Flu Shot within the past year.
• Comprehensive Immunization/Vaccination Records, including Covid19, Hep B, MMR, TDaP, and Varicella.
• Valid Driver's License along with auto-insurance.
