New York RN Care Manager

A company is looking for an RN- Care Manager (Bilingual) to provide care and support to members in the greater NYC area.

Key Responsibilities

Manage care planning, coordination, and health assessments for patients

Monitor the quality and effectiveness of services and track progress towards individual goals

Oversee patient transitions and facilitate discharge planning from hospitals or nursing homes

Required Qualifications, Training, and Education

Valid NYS RN License

Minimum of 3 years of relevant experience in Managed Care, Home Care, or Community settings

Associate's degree or higher required

Bilingual proficiency in languages such as Korean, Bengali, Cantonese, Mandarin, Spanish, Russian, or Creole

Ability to navigate and utilize various software and tools relevant to care management
