The Terraces at Los Altos, a distinguished HumanGood life plan community located in Los Altos, CA, is seeking a passionate and dedicated Charge Nurse RN - Admissions to join our skilled nursing team. In this vital role, you will manage the nursing needs of our residents while embracing our Philosophy for Person-Directed Care.
Full-time Position : $45-$54 / hr
Schedule : Sunday - Thursday - 11 AM - 7 PM
Qualifications for Success :
- Active RN license
- 1-2 years of experience as an Admissions RN
- At least 1 year of supervisory experience with RNs, LVNs, and CNAs
- Willingness to assist on the floor as needed
- Current CPR certification
- Preferred experience in senior care, skilled nursing, post-acute or sub-acute care
Key Responsibilities :
Provide comprehensive nursing care to residents and include them in decision-making processes.Engage residents in meaningful daily activities that support their personal rhythm of life.Assess residents during admission and continue evaluations throughout their stay, promoting independence in line with safety regulations.Lead and guide care partners, offering support and direction as needed.Conduct regular rounds, identifying conditions needing further evaluation and treatment.Document and report significant medical changes to team members, physicians, and family members as appropriate.React promptly to emergencies and ensure thorough documentation of interventions and outcomes.Administer nursing services, including medication, treatments, personal care, and other procedures.Ensure accurate recording and timely follow-through on physician orders, advocating for resident safety.Maintain complete documentation, including consent forms, care plans, and medication records.Participate actively in the community's Quality Assurance and Performance Improvement (QAPI) program.Collaborate with the interdisciplinary team to coordinate resident care plans.Build genuine relationships with residents by engaging in discussions about their histories and preferences.Maintain clear communication and provide exceptional service to residents, families, and team members.Assist with resident and family education while reporting any unusual changes to the Director of Nursing.What's in It for You?
