PRP Community Counselor

DPCS is a client-centered, family-driven psychiatric rehabilitation program located in Baltimore, MD. We are looking for a dedicated Psychiatric Rehabilitation Counselor (PRC) to join our team.

Preferably, our organization needs counselors that are willing to meet the client where they are. Due to the low functioning nature of our clients, an ideal candidate would be willing to travel to our office regularly for to meet the required encounter amount.

PRC’s are responsible for assuring continuity of care for clients in the psychiatric rehabilitation program and performing related duties as required. We are initially looking to hire for part-time with opportunities for growth and full-time. Our ideal candidate is a self-starter and has an entrepreneurial spirit. We are looking for a motivated, reliable, innovative staff member who truly cares about their clients, and is looking to make a difference in the community while collaborating and networking to make a difference.

Requirements

Ability to read, understand and follow detailed departmental procedures, and state and federal laws and regulations; learn practices of care management including evaluating needs of clients, referring clients to resources, and providing skill development for clients.

Must be a peoples person and comfortable meeting new people to discuss the program.

Job Summary

· Hours: Varies (Flexible)

· Attend at least 60 hours of training in the area of psychiatric rehabilitation prior to providing any direct services.

· Participate in ongoing quarterly trainings to equal at least 10 hours per year in the area of psychiatric rehabilitation.

· Participate in the initial assessment and development of the individual rehabilitation plan (IRP) with guidance from the Rehabilitation Specialist.

· Responsible for implementing the rehabilitation activities outlined in the individual’s IRP.

· Provide assistance to client in the area of: self-care skills, social skills, independent living skills, cultural interests, medication monitoring, health promotion, individual wellness self-management and recovery.

· Provide assistance with concrete tasks such as getting driver’s license or state ID, gaining employment, finding stable housing and apply for appropriate entitlement programs.

· Complete client assessments and IRP paperwork with assistance from Rehabilitation Specialist.

· Complete session notes for each client visit as well as monthly progress notes with assistance from Rehabilitation Specialist.

· Turn in all necessary paperwork by given deadlines.

· Participate in ongoing on-site and off-site sessions with client with a minimum of 4 sessions per month for minors and 6 sessions per month for adults.

· Observe and report to Rehabilitation Specialist observations of client’s behavior and progress towards goals.

· Collaborate with referring therapist and other treatment team members to ensure the client’s needs are being met. Seek outreach opportunities with the community to contribute to organizational growth

Ability to handle 15-30 client caseload. (Adults & youth)

What You’ll Do

  • Provide 1:1 and group psychiatric rehabilitation sessions (minimum of 4 per month for youth, 6 for adults).

  • Support clients in skill-building: self-care, social skills, independent living, health/wellness, housing, and employment.

  • Assist with practical needs such as obtaining an ID, driver’s license, or accessing community resources.

  • Collaborate with therapists, rehab specialists, and treatment teams to ensure quality care.

  • Complete progress notes, assessments, and IRP updates accurately and on time.

  • Manage a caseload of 15–30 clients (youth and/or adults).

· Case Management – Manages assigned caseload effectively, Develops a positive rapport with clients through the use of empathy, attending, reflection of feeling, immediacy, genuineness, concreteness, and positive regard; Display ability to work with diverse populations and services clients with varying needs and clinical presentation.

· Prevention & Intervention – Promotes optimal human development, wellness, and mental health through prevention, education, and advocacy activities.

· Ethics & Integrity – Demonstrates the ability to recognize his or her own limitations as a direct care worker and to seek supervision or refer clients when appropriate; maintains the confidentiality of client records and information; Avoids situations and actions considered inappropriate or which present a conflict of interest.

· Advocacy – Advocates for unique needs of each clients.

· Bonus opportunities monthly up to $1000 based on performance

· Community Linkage – Maintains information regarding community resources to make appropriate referrals.
