Facility Case Management Director (RN / LCSW / LMSW)

About the Role
The Facility Case Management Director is responsible for the overall leadership, management, and coordination of the hospital's Case Management Department. This role ensures compliance with organizational policies, regulatory requirements, and best practices while integrating patient care coordination and discharge planning across hospital departments and external partners.
This position provides strategic and operational leadership for all daily case management functions and is accountable for achieving performance, quality, and operational outcomes through strong interdisciplinary collaboration. The ideal candidate is an experienced acute care case management leader with executive presence, strong data-driven decision-making skills, and the ability to lead teams through change.
Key Responsibilities
- Direct and evaluate departmental operations, including :
- Case management model and workflows
- Staffing, skill mix, and FTE management
- Staff onboarding, education, and competency development
- Use of information technology and documentation systems
- Allocate resources to meet productivity, quality, and operational goals
- Provide oversight of all core Case Management functions and professional practice
- Ensure timely, high-quality care coordination and discharge planning with accurate documentation
- Partner with the Facility CFO and Division Case Management Leadership to achieve strategic goals
- Monitor performance metrics, analyze data, and lead sustainable performance improvement initiatives
- Ensure compliance with organizational policies, procedures, and regulatory standards
- Build strong working relationships with key stakeholders, including :
- Chief Medical Officer (CMO)
- Chief Nursing Officer (CNO)
- Ancillary Service Leaders
- Ethics & Compliance and Legal teams
- Lead with integrity by adhering to the organization's Code of Conduct, Mission, and Values
- Perform additional duties as assigned
Qualifications
Required :
Bachelor's degreeActive licensure as one of the following :Registered Nurse (RN)Licensed Clinical Social Worker (LCSW)Licensed Master Social Worker (LMSW)Minimum of 3 years of recent acute care hospital Case Management experienceMinimum of 2 years of Case Management leadership experiencePreferred :
