Care is based on nursing theory and research for a designated group of patients and their families. Delivers patient care by applying the nursing process based on theory, research, and evidence-based practice. Assesses and evaluates patient care needs while applying critical thinking skills in patient management. Integrates relevant assessment and intervention skills into the delivery of nursing care. Collaborates with the interdisciplinary healthcare team to develop, implement, and evaluate patient care plans. Prioritizes all aspects of patient care including teaching, rounding, coaching, and post-hospital care planning. Educates patients and caregivers about their plan of care and promotes health and disease prevention. Communicates effectively and professionally with patients, families, and all members of the interdisciplinary team. Manages assignments within the Care Delivery Model and allocates resources effectively. Role models relationship-centered care principles through various patient interactions. Considers patient values, preferences, cultural diversity, and expressed needs in care delivery. Establishes and maintains therapeutic relationships with patients and their families. Uses evidence-based practices to enhance understanding of patient perceptions of care. Initiates service recovery for patients and family members who have concerns. Demonstrates respect, compassion, empathy, and active listening in all interactions. Supports staff in applying evidence-based practices to improve an understanding of patient care perceptions. Conducts purposeful hourly rounding on assigned patients addressing key needs. Practices safe and efficient patient-centered care, identifying barriers and opportunities for improvement. Demonstrates clinical skills and knowledge vital to quality and safety in patient care. Implements process improvement strategies based on institutional and evidence-based standards. Engages in peer and institutional review processes to enhance quality and safety practices. Collaborates with interdisciplinary teams to create and implement quality improvement and evidence-based activities. Contributes to necessary compliance measures related to quality and safety practices. Implements principles of high reliability to ensure a safe patient environment. Utilizes institutional information technology to document nursing practice for performance improvement. Performs charge nurse duties as assigned. Coordinates with management to ensure appropriate utilization of supplies and inventory for patient care. Escalates any material or staffing resource deficits to the appropriate management. Demonstrates patient-focused approaches to resource management in patient care. Documents patient care accurately in accordance with legal and regulatory standards. Facilitates patient throughput on a shift basis. Contributes to the environment of care supporting clinical colleagues and members of the healthcare team. Advances clinical competence in nursing practice to enhance professional development. Projects a professional image and communicates with empathy and respect.
