Concordia Visiting Nurses is a Medicare-certified, state licensed, Medicaid approved home health agency operating in nine Western Pennsylvania counties. For well over a decade, Concordia Visiting Nurses has provided high-quality skilled nursing, palliative care, and a variety of therapy services to our patients. Clinical Scheduler :

Are you a passionate problem-solver with a knack for crafting elegant solutions? A Clinical Scheduler is being sought to join a rapidly growing team and shape the future of Concordia Visiting Nurses.

What you'll do as a Clinical Scheduler :

  • Schedule Management : Creating and managing daily and weekly schedules for clinical staff, ensuring appropriate staffing levels based on client needs, preferences, and availability.
  • Client & Caregiver Communication : Effectively communicating schedule updates, changes, and important information to clients, families, and clinical staff.
  • Problem Solving & Adaptability : Responding proactively to last-minute changes, call-offs, and urgent client needs, finding efficient solutions to maintain continuity of care.
  • Documentation & Compliance : Maintaining accurate records of schedules, authorizations, and client information within the scheduling software and electronic health records (EHR) systems, adhering to all HIPAA regulations and compliance standards.
  • Collaboration : Working closely with the Clinical Manager, Intake team, and other staff members to coordinate admissions, initial visits, and ensure smooth transitions of care.
  • Customer Service : Demonstrating a strong focus on customer service, ensuring all interactions are professional, empathetic, and patient-centered.

  • Personable & Empathetic : A friendly and positive demeanor with exceptional interpersonal skills, fostering trust and rapport with clients and caregivers.
  • Highly Organized : Excellent organizational skills with meticulous attention to detail to manage complex schedules and multiple priorities effectively.
  • Strong Communication : Clear and effective verbal and written communication skills for interacting with diverse individuals.
  • Problem-Solver & Proactive : Ability to anticipate potential scheduling conflicts and find innovative solutions to ensure seamless care delivery.
  • Technical Proficiency : Comfortable using scheduling software and Electronic Health Records (EHR) systems.
  • High school diploma or GED equivalent.
  • Knowledge of medical terminology and home health care operations is a plus.
