Epic Health Partners, LLC is seeking compassionate and motivated QMHP-registered professionals to join our team as Crisis Case Managers. This full-time position includes a blend of in-office responsibilities and field-based services, providing crisis response, community stabilization, and case management support.

Key Responsibilities :

• Deliver direct crisis intervention and stabilization services in the community.
• Support the completion of initial assessments and assist with the collaborative development of Individualized Service Plans (ISPs) under the direction of a licensed professional.
• Utilize de-escalation techniques and provide basic mental health education to support individual stabilization.
• Accurately document all service notes and submit all documentation within 24 hours of service delivery.
• Ensure all documentation, including required signatures, is uploaded timely and charts remain 100% compliant.
• Assist with coordination of care through referrals and discharge planning to connect individuals with appropriate internal and external behavioral health resources.
• Provide case management support for individuals enrolled in Mental Health Skill-Building (MHSB) services during periods of lower crisis response activity.
• Perform other duties as assigned.
