Job Summary :

The Travel RN Case Manager is responsible for coordinating and managing patient care across the continuum to ensure optimal clinical outcomes, efficient resource utilization, and safe transitions of care. The RN Case Manager serves as a liaison between patients, healthcare providers, and community resources to facilitate high-quality, patient-centered care in an acute care setting.

Job Summary : \n\nThe Travel RN Case Manager is responsible for coordinating and managing patient care across the continuum to ensure optimal clinical outcomes, efficient resource utilization, and safe transitions of care. The RN Case Manager serves as a liaison between patients, healthcare providers, and community resources to facilitate high-quality, patient-centered care in an acute care setting.\n\n\nKey Responsibilities : \n\nPerform comprehensive assessments and develop individualized care plans for assigned patients.\nCoordinate care and discharge planning in collaboration with physicians, nurses, and interdisciplinary teams.\nMonitor patient progress and ensure continuity of care through appropriate referrals and follow-ups.\nEvaluate medical necessity, length of stay, and utilization of healthcare resources.\nServe as a patient advocate to promote access to services and quality outcomes.\nProvide education and support to patients and families regarding post-discharge needs and available community resources.\nMaintain accurate documentation in accordance with facility standards and regulatory requirements.\nFacilitate transitions of care between acute, post-acute, and community settings.\nCollaborate with insurance providers to ensure proper authorization and reimbursement.\n\nRequirements : \n\nExperience : Minimum 2 years of recent acute medical Case Management experience performing Care Coordination (Required).\nLicensure : Active Registered Nurse (RN) license in the state of assignment.\n\nCertifications : \n\nBLS – Basic Life Support (Required).\nACM or CCM (Accredited Case Manager / Certified Case Manager) certification preferred but not required.\nFloating Expectation : Must be willing to float to Laguna Facility as needed.\nRatios : 1 : 25 (Patients per RN Case Manager).\n\nPreferred Qualifications : \n\nExperience with acute hospital discharge planning and utilization review.\nStrong knowledge of Medicare, Medicaid, and insurance authorization processes.\nExcellent communication, critical thinking, and organizational skills.\nExperience with EMR systems and case management software.
