The required availability for this position is

Monday–Friday, full-time field schedule

, completing approximately

2–4 UAS assessments per day

Field Registered Nurse (RN) – LTSS / UAS Assessor Responsibilities

Conduct in-home and site-based UAS assessments for long-term care members

Assess, coordinate, and manage medical and supportive services for complex / high-acuity populations

Develop, implement, and update individualized care / service plans

Coordinate care with members, families, caregivers, providers, and community resources

Monitor member condition, progress toward goals, and need for changes in level of care

Educate members and caregivers on benefits, services, disease processes, and care options

Document all care management activities in compliance with state and federal regulations

Act as a liaison and advocate between members, providers, and service agencies

Support onboarding and precepting of new clinical staff as needed

Perform other duties as assigned
