The Optum at Home program delivers a longitudinal, integrated care approach that coordinates clinical care for members in their own homes. We focus on personalized interventions tailored to individual needs, collaborating with an Interdisciplinary Care Team that includes clinicians, Primary Care Providers, and other professionals.

This field-based position serves members in the greater Washington D.C. area.

Expect to spend approximately 50-75% of your time visiting members in their homes or in long-term care facilities. We are looking for someone who is flexible, adaptable, and patient in all situations.

Standard Hours : Monday - Friday, during normal daytime business hours (no on-call, weekends, or holidays required).

Primary Responsibilities :

  • Assess, plan, and implement individualized care management interventions.
  • Develop and track care plans, being the primary contact for members.
  • Communicate effectively with all stakeholders to ensure coordinated care is promptly provided.
  • Advocate for members and families to represent their needs and preferences.
  • Identify resources and interventions to address gaps in care, including psychosocial and transportation needs.
  • Document care plans accurately in EHR systems.
  • Engage consumers by building rapport and fostering trust.
  • Collaborate with primary providers to align care goals and ensure effective coordination.
  • Support members with advanced care planning and regular reassessment of their care plans.
  • Operate within legal and regulatory requirements.
  • Utilize evidence-based guidelines to inform clinical decisions.
  • Conduct outbound and inbound calls to assess health statuses and identify treatment plan barriers.
  • Educate members on self-management strategies.
  • Refer members to external resources as needed.
  • Maintain a continuum of quality care through open communication with members.
  • Assist members with condition education, medication reviews, and connections to resources.

Required Qualifications :

  • Current unrestricted Registered Nurse license in Washington D.C., or ability to obtain within 90 days of hire.
  • Certified in Basic Life Support.
  • 2+ years of experience with MS Word, Excel, and Outlook.
  • 1+ years of experience in post-acute care, such as long-term care.
  • 1+ years of clinical case management experience.
  • 1+ years of experience using an Electronic Medical Record.
  • Valid Driver's License and access to reliable transportation.
  • Ability to work in a field-based capacity.
  • Reside within 50 miles of Washington, DC.
  • Preferred Qualifications :

  • Certified Case Management (CCM).
  • 1+ years of experience working with the geriatric population.
  • 1+ years of experience with Long Term Services and Supports (LTSS).
  • 1+ years of Home and Community Based Services (HCBS) experience.
  • Field experience visiting members in their homes.
  • Experience creating care plans.
  • Background in managing complex medical or behavioral needs.
  • Our pay scales reflect various factors including local labor markets, education, experience, and certifications. In addition to salary, we offer benefits such as a comprehensive package, incentive programs, equity stock purchase options, and 401k contributions. The salary range is $58,800 to $105,000 annually for full-time employment.
