[job_card.job_description]Assist nursing staff with simple medical procedures such as suppositories, enemas, treatments, post-surgical care, and post-mortem care. Monitor and record vital signs (temperature, pulse, respiration) and collect urine, stool, or sputum samples when required. Track intake / output and document all care provided in medical records and worksheets. Maintain cleanliness and sanitary conditions, including changing linens and ensuring living areas are neat and safe. Support emergency response efforts in accordance with established protocols. Transport residents to appointments within or outside the facility. Provide basic clerical support such as answering phones and filing. Participate in Arizona Management System (AMS) processes, including Huddle Boards, Problem Solving, Job Breakdown Sheets, Success Tracking, and Standardized Work. Contribute to Quality Assurance and Performance Improvement (QAPI) initiatives as mandated by CMS. Para-professional nursing care principles, practices, and techniques. State and agency regulations governing CNA practice. Safe use and maintenance of medical equipment. Standards and procedures for maintaining a safe, therapeutic, and hygienic environment. Applying para-professional nursing methods and interpreting related regulations. Maintaining, using, and cleaning medical equipment. Recording, filing, and retrieving medical records accurately. Effective verbal and written communication. Establish and maintain positive working relationships. Learn, apply, and support Arizona Management System (AMS) methodologies. Active Certified Nursing Assistant (CNA) certification in Arizona.

Location : Phoenix, Arizona
Shift : 6 : 00 AM - 6 : 30 PM (12-hour shifts)
Position Summary
The Certified Nursing Assistant (CNA) provides essential hands-on care, support, and monitoring for residents in a long-term care and skilled nursing setting. This role works closely with nursing staff to ensure safe, dignified, and compassionate care while supporting both clinical and daily living needs of Veterans / Members.
Responsibilities
Direct Resident Care
- Assist residents with transfers, ambulation, bathing, dressing, toileting, eating, and overall personal hygiene.
- Follow established procedures and use appropriate devices to maintain or improve residents' mobility and limb strength.
- Support residents during dining, including meal transport, explaining daily menu choices, coordinating with dining staff, and providing mealtime assistance.
- Respond promptly to call lights and verbal requests in accordance with the Resident Bill of Rights.
