Physical Therapist - Days 8 Hours

Job Description

The physical therapist will accurately assess, differentially diagnose, and identify deficits in a patient's ability to move and perform functional activities in their daily lives. They will develop an individualized treatment plan with patient-centered goals aimed at reducing pain, restoring function, and preventing disability while focusing on mobility preservation.

Responsibilities

• Perform patient evaluations by obtaining history, conducting relevant systems reviews, and administering specific tests and measures to gather data.
• Establish individualized plans of care and select appropriate treatment techniques that target identified impairments in accordance with physician orders.
• Conduct ongoing reassessments of the patient and their response to treatments.
• Initiate and manage discharge plans beginning at the first visit and throughout treatment.
• Supervise and delegate tasks to ancillary staff and students in compliance with state laws and department policies.
• Complete documentation accurately and within established timeframes, adhering to department policies and insurance regulations.
• Communicate effectively with patients, families, and healthcare team members to maximize the benefits of care.
• Assess the educational needs of patients and provide appropriate education to meet those needs.
• Participate in professional development through educational programs, workshops, and literature review.
• Engage in quality improvement activities and maintain safety standards for patients and staff.
