Mosholu Parkway Nursing Home is a caring, resident-focused long-term care and rehabilitation facility located in the heart of the Bronx. We are currently seeking a dedicated and empathetic Social Worker to join our team and help ensure the emotional and social well-being of our residents and their families.
The Social Worker is responsible for assisting residents and their families with psychosocial needs, discharge planning, and coordination of care throughout their stay. This role involves close collaboration with nursing, therapy, and administrative teams to ensure that residents receive comprehensive, person-centered support.
Conduct psychosocial assessments for new residents and develop individualized care plans.
Provide emotional support and counseling to residents and families.
Facilitate care planning and discharge coordination, ensuring smooth transitions of care.
Maintain accurate and timely documentation in compliance with federal, state, and facility standards.
Advocate for residents’ rights and ensure their social, emotional, and psychological needs are met.
Participate in interdisciplinary team meetings and contribute to resident care discussions.
Support families with adjustment to nursing home placement and ongoing care issues.
Assist with crisis intervention, conflict resolution, and behavioral management when needed.
Bachelor’s or Master’s Degree in Social Work (BSW/MSW) from an accredited program required.
Current New York State licensure (LMSW or LCSW preferred).
Experience in a skilled nursing or long-term care setting strongly preferred.
Strong interpersonal and communication skills.
Ability to manage multiple priorities and maintain professionalism in a fast-paced environment.
