[job_card.job_description]Experience : A minimum of 1,000 hours (approximately 6 months full-time or 12 months part-time) of experience working with geriatric or chronically ill individuals is preferred. Exceptions made for recently licensed CNAs, HHAs, or those providing companionship and housekeeping only. Certifications : CNA or HHA certification is preferred but not required. Health and Safety : Proof of current TB testing, ability to safely transfer clients, and compliance with infection control protocols. Transportation : Reliable transportation, valid driver's license, and automobile insurance required for client transport. Background Check : Must be able to pass a criminal background check (national and state-required). Communication : Excellent communication skills for interacting with clients, family members, and team members. Care Plan Implementation : Follow personalized care plans to ensure clients receive the appropriate support and assistance. Client Support : Provide companionship, meal preparation, housekeeping, transportation, and personal care (bathing, dressing, toileting, feeding). Communication and Advocacy : Report any changes in client condition to the Care Manager and advocate for client needs. Safety and Compliance : Adhere to safety protocols and infection control practices; notify the office if additional supplies are needed. Timekeeping and Documentation : Accurately log hours, tasks, and client status using ClearCare Telephony system. Professionalism : Maintain a professional appearance and conduct, adhering to company policies and dress code. Ability to assist with client transfers and repositioning using proper techniques and equipment. Reliable transportation to clients' residences.
