Role : Clinical Documentation Medical Professional (Onsite)
Location : Irving, TX 75039
Pay Rate : $45–$48 / hour
Schedule : Monday–Friday, 8 hours / day
Duration : 3-month contract to start (with strong possibility of extension)
Position Overview :
We are seeking a Clinical Documentation RN to join our onsite team in Irving, TX. This role focuses on reviewing and documenting home health recertifications, ensuring compliance with CMS criteria, and coordinating with home health agencies to support patient care. While there is no direct patient contact, this position plays a critical role in ensuring high-quality care transitions and accurate clinical documentation.
Key Responsibilities :
Conduct a high volume of telephone outreach to home health agencies.
Review home health recertifications and ensure patients meet CMS criteria for ongoing services.
Perform extensive documentation in EPIC to maintain accurate, timely records.
Collaborate with physicians, clinics, hospitals, caregivers, and community healthcare providers to support care transitions.
Serve as a patient advocate through coordination and communication with providers and agencies.
Make recommendations to primary care providers regarding continuation of services.
Qualifications (Must Haves) :
Bachelor’s Degree in Nursing (proof of education required at submission).
Active RN license (submit with full Nursys report : https : / / www.nursys.com / LQC / LQCTerms.aspx).
Minimum 3 years of nursing experience in a clinical setting.
At least 2 years of home health experience .
2–3 years of managed care and / or care management experience .
Strong knowledge of CMS criteria for home health.
Experience with EPIC preferred.
