About the Role:
As an Occupational Therapist, youll provide personalized therapy in a home setting, helping patients regain independence in daily living activities through customized treatment plans and adaptive strategies.

Key Responsibilities:
Conduct comprehensive patient assessments and establish realistic therapy goals.

• Develop personalized treatment plans focused on ADLs, motor skills, cognitive function, and environmental modifications.
• Provide hands-on occupational therapy services, helping patients increase mobility and functionality.
• Educate patients and caregivers on adaptive techniques, energy conservation, and safety strategies.
• Collaborate with nurses, physical therapists, and other healthcare professionals to provide comprehensive care.
• Maintain accurate and timely documentation following regulatory standards.
• Stay current on home health regulations and evidence-based therapy techniques.

Qualifications:

• Valid State License as an Occupational Therapist
• Minimum 1 Year of Experience in home health or acute care
• Strong clinical skills with a patient-first mindset
• CPR Certification (Required)
• Excellent communication & interpersonal abilities
• Reliable transportation for home visits
• Understanding of Medicare home health regulations & documentation
