Pinnacle Home Care Floridas largest Medicare-certified home health provider has been delivering high-quality patient-centered care for over two decades and were looking for Registered Nurses to join our award-winning team. This is a fulltime weekend Baylor position.

Key Responsibilities

• Design and implement patient-specific care plans focused on achieving optimal recovery and enhancing overall health.
• Provide high-quality nursing care in alignment with the patients care plan including comprehensive health and psychosocial assessments to support recovery.
• Monitor patients conditions and coordinate health promotion prevention and direct care.
• Maintain detailed and accurate clinical records including all required OASIS documentation.
• Communicate effectively with clinical managers and colleagues to ensure high-quality care.
• Provide training and support to patients and their families / caregivers.
• Function as a key point of contact between patients and community health resources.
• Work alongside physicians and discharge planners to provide health education to patients and caregivers.
• Confirm daily visit schedules to ensure seamless coordination with other staff.
• Demonstrate awareness of the agencys Quality Assurance and Performance Improvement (QAPI) program and participate as required.
• Understand and adhere to the agencys process for managing and addressing customer complaints.
• Maintain acute awareness of patient rights ensuring these rights are upheld in accordance with the agencys service agreements and admission policies.
• Understand the agencys policies regarding appropriate patient discharge and reasons for discharge.
